At Access Care, case management is at the heart of how we support older adults and pediatrics in navigating their healthcare journey. From there, we create personalized care plans tailored to the individual’s goals, preferences, and medical requirements. We coordinate across healthcare providers, community resources, and family members to ensure attendant care and/or adult day health care services are delivered seamlessly and efficiently.
